According to a report published by Cureus, this condition can manifest as apparent severe hyperkalemia—dangerously high potassium levels in the blood—despite a lack of clinical symptoms or abnormalities on an electrocardiogram (ECG).

The Role of Extreme Leukocytosis

A specific cause of this phenomenon is extreme leukocytosis, a condition characterized by an abnormally high number of white blood cells in the blood.

In a case reported by Cureus, a patient presented with markedly elevated serum potassium measurements. However, the patient did not exhibit the typical clinical symptoms associated with severe hyperkalemia, nor did their ECG show the expected abnormalities.

The final diagnosis in this instance was pseudohyperkalemia secondary to extreme leukocytosis. The elevation in potassium levels was attributed to in vitro leukocyte lysis, meaning the white blood cells ruptured during the sample collection or processing phase, releasing potassium into the serum.

Diagnostic Pitfalls and Clinical Indicators

Distinguishing between true hyperkalemia and pseudohyperkalemia is critical because the treatment for severe hyperkalemia can be invasive and risky if the patient’s actual potassium levels are normal.

Research published via ScienceDirect suggests that pseudohyperkalemia should be a primary consideration for clinicians when certain conditions coincide, including:

  • The presence of leukocytosis alongside unexplained hyperkalemia.
  • Patients who maintain normal electrocardiograms despite high potassium readings.
  • Patients who do not have concomitant acidosis or kidney failure, which are common drivers of true hyperkalemia.

Scientific Context and Hypotheses

The medical community has explored various hypotheses regarding why extreme leukocytosis leads to falsely elevated potassium levels.

One theory regarding these falsely elevated levels dates back to 1975, as noted in a case report available through PubMed Central (PMC).

Because the potassium release occurs in the test tube (in vitro) rather than inside the patient’s body (in vivo), the patient is not actually suffering from the systemic effects of high potassium, which is why the heart’s electrical activity remains normal on an ECG.
