With its 170 employees, the LNA⁢ Santé home hospital (HAD) ⁣in Loir-et-Cher supports more than two hundred ⁢patients with various pathologies every day. ⁣Keen to respond to the evolving needs of society, the structure regularly ⁢develops new care pathways. “As early‍ as 2022, the idea of creating a rehabilitation and re-adaptation pathway emerged within HADs throughout France. The objective⁤ is notably to address the lack of rehabilitation specialists and specialized structures nationally and to offer patients a rehabilitation pathway while remaining at home,” explains Dr. Maria Lapadat, a physician specializing in physical medicine ⁣and⁣ rehabilitation at HAD⁣ Loir-et-Cher.

In Loir-et-Cher, this pathway officially launched in November 2023. Two years after its launch,a team of around ten professionals has followed 260 patients in the past year alone.⁤ “The specificity of this pathway lies in its multidisciplinary care,” emphasizes Laura Maréchal, coordinating nurse. “the patient benefits from 5 rehabilitation sessions⁣ per week, at home, carried out by ⁢a⁢ minimum of 3 rehabilitation specialists from ⁤different professions.”

The pathologies treated are numerous: orthopedic (post-orthopedic surgery, polytrauma, post-fractures with resumption of support / verticalization…), neurological ‍(sequelae ⁢of stroke, ParkinsonS disease, multiple sclerosis…), ⁣oncology… Consequently, ⁤various⁤ specialists⁤ (physiotherapists, neuropsychologists, dietitians, occupational ‍therapists, adapted physical activity instructors, psychomotor therapists and speech therapists)‍ intervene within this pathway ‍in connection with the other professions of the HAD (psychologists, social workers, nurses…).

A streamlined Care Pathway

To ‍guarantee optimal follow-up, the HAD relies on a rigorous process.”Each care pathway begins with an assessment at the patient’s home to build a personalized care plan, adapted to their pathology and needs,” details Dr. Maria Lapadat. “We then define the care to⁢ be implemented and mobilize the ‍necessary professionals. A ‍multidisciplinary meeting is held regularly for each patient in ⁣order⁣ to monitor their ‍progress and adjust care if necessary.”

Federal Student Loan Forgiveness Programs in 2026

As of January 26, 2026, several federal student loan forgiveness programs are available to eligible borrowers,⁢ offering pathways to debt cancellation or reduced payments. These programs are administered by the U.S. Department of Education and include options based on income,employment,and loan type.

Income-Driven Repayment (IDR) Forgiveness

Income-Driven Repayment (IDR) plans cap monthly payments based on a borrower’s income and family ⁤size. After a set number of ⁣years ⁤of qualifying payments,‍ the remaining loan balance is forgiven. The specific timeframe for forgiveness varies by plan.

  • SAVE Plan (Saving on a Valuable Education): Replaced the REPAYE plan in July 2024. Borrowers on ⁣the SAVE plan recieve forgiveness after 20⁣ years for undergraduate loans and 25 years for graduate loans.The plan also includes a weighted average calculation for borrowers with multiple loan types, possibly lowering monthly payments.

On April 22, 2024, the Department of Education announced adjustments to IDR plans to address ancient inaccuracies in payment ⁣counting. These ⁣adjustments, stemming from‍ a settlement in Sweet v.Cardona, will ⁢provide credit toward forgiveness for past periods ⁣of forbearance, deferment, and certain repayment statuses. As of December 2025, approximately 30 million borrowers were eligible for some form‍ of adjustment, resulting in an estimated $28 billion in forgiveness. public Service Loan Forgiveness (PSLF)

The Public Service Loan Forgiveness (PSLF) program forgives the remaining balance on Direct Loans after 120 qualifying monthly payments⁢ while working full-time for a qualifying employer. Qualifying employers include government organizations, non-profits, and certain⁤ public service roles.

In ⁢October 2021, the Department of Education announced‍ a temporary waiver allowing borrowers to receive credit for past payments that wouldn’t have previously qualified‍ under PSLF rules.
