Resolving Thyroid Eye Disease Diplopia with Myopic Cataract Surgery

A report published in Cureus indicates that intentional myopic targeting during unilateral cataract surgery can lead to the resolution of diplopia, or double vision, in patients with thyroid eye disease (TED).

Thyroid eye disease is an autoimmune inflammatory disorder affecting the orbital tissues. It is most commonly associated with Graves’ hyperthyroidism, though it can also occur in individuals in euthyroid or subclinical states.

Understanding Thyroid Eye Disease and Diplopia

TED affects approximately 25% to 50% of the population with Graves’ disease. The condition is characterized by an autoimmune reaction that causes inflammation and swelling in the eye muscles.

Common symptoms associated with the disease include redness, pain, and bulging eyes. One of the most prevalent complications is diplopia, which occurs when the eyes produce overlapping images.

This double vision is caused by the thickening and scarring of the extraocular muscles, which leads to the misalignment of the eyes, a condition known as strabismus. The resulting visual impairment can make daily activities, including driving, reading, and using electronic devices, difficult.

Several factors contribute to the development and severity of TED, including:

• Genetics and a family history of autoimmune thyroid disorders.
• Smoking, which is identified as a major factor that worsens the severity of the condition.
• Environmental triggers, such as stress, air pollutants, and exposure to radioactivity.

Surgical Interventions and Outcomes

While the Cureus report highlights the resolution of diplopia through intentional myopic targeting during unilateral cataract surgery, other medical literature suggests that the relationship between cataract surgery and TED is complex.

Some patients may experience the aggravation of thyroid orbitopathy following eye surgery. A case series published on May 24, 2024, in the Journal of Clinical Research and Ophthalmology noted that cataract surgery can be associated with devastating visual consequences in some instances of thyroid orbitopathy.

In another instance reported on October 22, 2025, a 67-year-old Chinese-Malaysian man developed eyelid swelling, redness, and blurred vision two weeks after an uncomplicated left-eye cataract surgery.

His condition worsened over six months, with visual acuity declining from 6/12 to 6/30. He subsequently presented with proptosis, diplopia, and optic nerve dysfunction, highlighting that TED can sometimes emerge or mimic routine postoperative inflammation following ocular surgery.

Clinical Considerations

The variability in outcomes suggests that the presence of TED must be carefully considered when planning and evaluating postoperative inflammation. Because TED can mimic standard postoperative responses, diagnosis may be delayed if the symptoms are initially treated as common complications.

Medical professionals are encouraged to consider TED in patients who present with atypical orbital inflammation or persistent diplopia following cataract procedures.
