Revolutionary Two-Medication Combo Pill Lowers Blood Pressure in Indian Adults: A Breakthrough in Hypertension Treatment

Key Findings from the Study

In Chicago, on Nov. 17, 2024, researchers presented findings on effective blood pressure treatments at the American Heart Association’s Scientific Sessions. High blood pressure is a major health issue, impacting 300 million people in India and nearly half of adults in the U.S. Current guidelines suggest using combinations of blood pressure medications. Yet, previous research has not focused on which combinations work best for South Asian populations.

Dr. Dorairaj Prabhakaran, study lead and a cardiologist, emphasized that India faces a significant hypertension burden. Effective blood pressure management is crucial to reduce heart disease risks.

This clinical trial, called TOPSPIN, targeted hypertension in South Asians. Nearly 2,000 adults in India participated, randomly assigned to one of three treatment groups. Each group received a single pill with two well-known blood pressure medications.

The medications included:

  • Perindopril: a renin-angiotensin enzyme inhibitor that helps prevent artery narrowing.
  • Amlodipine: a calcium channel blocker that stops calcium from entering heart muscles and arteries.
  • Indapamide: a diuretic that helps the body eliminate extra salt and water.

Results after Six Months:

  • All combinations equally reduced blood pressure. About 70% reached below 140/90 mm Hg.
  • Over 40% achieved a stricter target of under 130/80 mm Hg.
  • Participants experienced an average reduction in blood pressure by about 30/14 mm Hg in doctor’s offices and 14/8 mm Hg with ambulatory monitors.
  • Fewer than 3% of participants discontinued treatment due to side effects, indicating good medication tolerance.
  • The group taking the amlodipine-perindopril combination also saw a drop in fasting blood sugar levels.

Dr. Prabhakaran expressed reassurance in the effectiveness of all three medication combinations. This study contrasts with previous findings from the CREOLE study, which found amlodipine-based combinations more effective for Black adults in Sub-Saharan Africa.

The trial included a wide demographic, accounting for various ages, genders, and diabetes status, making its findings applicable beyond India, particularly significant for over five million South Asians living in the U.S.


Study Background

  • Conducted from 2022 to 2024, involving 1,981 participants aged 30-79, with a median age of 52 and 42% women, across 35 hospitals in India.
  • 55% were already on some blood pressure medications upon entering the study.
  • Participants with cardiovascular or kidney diseases were excluded.

Conclusion

This study highlights the effectiveness and safety of combining blood pressure medications into a single pill for South Asian adults. Simplifying medication regimens can improve adherence and control, aligning with current recommendations from health professionals.
