New research emphasizes the importance of human challenge studies (HICs) in understanding respiratory syncytial virus (RSV) and developing effective treatments. A systematic review and meta-analysis published in The Lancet highlights the role of these studies in evaluating viral load (VL) and symptom progression.

RSV is a major concern for vulnerable populations, including older adults and those with conditions like asthma and heart failure. The Centers for Disease Control and Prevention (CDC) estimates that RSV leads to 100,000 to 150,000 hospitalizations annually among adults aged 60 and older.
HICs involve intentionally exposing participants to a pathogen to assess the efficacy of potential treatments or vaccines. Researchers conducted a meta-analysis of double-blind, placebo-controlled RSV HICs from 1990 to August 2023. The analysis focused on viral load kinetics and symptom dynamics, evaluating the impact of experimental drugs.
The study found that the average reduction in viral load area under the curve (AUC) was 54%, while the total symptom score (TSS) AUC showed a 76% reduction. Peak TSS occurred at the same time as peak VL for RSV,differing from SARS-CoV-2 and influenza.
Researchers noted that variations in infection rates among participants and other factors could introduce heterogeneity. These factors include prior RSV exposure, immunity levels, and seasonal differences.
