Social workers in Santa Clara County have been placed on administrative leave following the death of 2-year-old Jaxon, known as “Baby Jaxon,” who died in foster care on April 9, 2026, after suffering repeated physical and sexual abuse.

The child was placed in foster care in February 2026 with a 40-year-old woman identified as a relative and the mother of a 17-year-old boy now charged in connection with the toddler’s death. Authorities confirmed the caregiver had a known criminal history that was not adequately screened prior to placement.

Santa Clara County District Attorney Jeff Rosen cited “serious lapses” by the Department of Family and Children’s Services for failing to exercise proper oversight in the case. The agency had been under state oversight for 18 months prior to Jaxon’s death due to at least two previous child fatalities in the system, with reforms mandated by the California Department of Social Services.

The 17-year-old cousin appeared in juvenile court on April 14, 2026, facing six counts of sexual assault on a child. Prosecutors stated they are awaiting further investigative findings before determining whether to pursue a murder charge in connection with Jaxon’s death.

Family members, including Jaxon’s maternal aunt Riley Wallace, reported concerns to county officials after the child’s mother died in July 2025 and his father was deemed unable to care for him. Wallace said the family had been involved with the child welfare agency since Jaxon’s birth and expressed frustration that warnings about his living situation were not acted upon.

A vigil was held on Tuesday, April 21, 2026, at Juvenile Hall in San Jose to honor Jaxon and call for systemic changes to the foster care system. Wallace described Jaxon as a “very bubbly, happy kid” with a “contagious laugh” who was “always smiling” and “full of love and happiness.”

According to Wallace, Jaxon was born premature and likely affected by fetal alcohol syndrome and autism. She stated the placement with relatives approved by the county “was not the best life that he needed to be given.”

The Santa Clara County Social Services Agency and the Department of Family and Children’s Services defended their family preservation policies during a Board of Supervisors meeting on December 19, 2023, but acknowledged ongoing scrutiny following Jaxon’s death, marking the third child death in as many years under the agency’s care.

County Supervisor Sylvia Arenas and Steve Baron of the Child Abuse Prevention Council have called for an independent investigation by the state’s Department of Social Services to determine whether county actions contributed to Jaxon’s death.
