• In Davenport, J Ivy At Davenport was cited on December 16, 2025, for failing to ensure the facility was free from accident hazards and provided adequate supervision to prevent accidents.
• In Shenandoah, J Garden View Care Center received two citations on December 8, 2025: one for failing to protect residents from all types of abuse, including physical, mental, sexual abuse, physical punishment, and neglect, and another for failing to respond appropriately to alleged violations.
• In West Branch, J Crestview Specialty Care was cited on December 3, 2025, for failing to provide appropriate pressure ulcer care and prevent new ulcers from developing.

• In Independence, Rehabilitation Centers Of Independence West Campus was cited on November 6, 2025, for failing to ensure the facility was free from accident hazards and provided adequate supervision to prevent accidents.
