Shingles Vaccine Linked to Lower Dementia Risk in New Study
A large-scale analysis suggests a potential link between the shingles vaccine and a reduced risk of dementia in older adults. Recent research indicates that the vaccine could become a valuable tool in the ongoing effort to combat cognitive decline, notably that associated wiht Alzheimer’s disease.
Shingles Vaccine and Dementia: A Promising Connection
A vaccine targeting shingles, also known as herpes zoster, has been associated with a 20% decrease in dementia risk among older individuals, according to a study conducted in Wales. The findings contribute to a growing body of evidence suggesting a protective effect of the shingles vaccine against dementia, although researchers emphasize the need for further investigation to establish a definitive causal relationship.
The study, published in the journal
Nature, involved approximately 283,000 older adults in Wales. The unique design of the Welsh vaccination programme, which initially included 79-year-olds in 2013 but excluded those 80 and older due to limited vaccine availability, allowed researchers to compare vaccinated and unvaccinated groups.
Researchers from the United States, germany, and Austria compared individuals who turned 80 shortly before and after the eligibility cutoff. Approximately half of those eligible received the vaccine.
After a seven-year follow-up period, about one in eight unvaccinated individuals developed dementia.though, those who received the shingles vaccine exhibited a 20% lower risk of receiving a dementia diagnosis. The protective effect appeared more pronounced in women than in men.
“It is indeed a truly remarkable finding,” said Dr. Pascal Geldsetzer, lead author of the study and assistant professor at stanford University, in a statement. “This powerful protective signal was present no matter how we analyzed the data.”
Comparing shingles Vaccines
Previous research,including a 2024 analysis published in
Nature Medicine,also indicated a reduced dementia risk among individuals who received a newer version of the shingles vaccine. These two vaccines differ in their composition. The newer vaccine contains a protein from the varicella-zoster virus, responsible for both chickenpox and shingles, while the vaccine used in the Welsh study employed an attenuated, or weakened, version of the live virus.
In 2023, the United Kingdom transitioned from the live virus vaccine to the newer version produced by GlaxoSmithKline (GSK), citing its potential for longer-lasting protection against shingles. Shingles, characterized by a painful rash with blisters, can occur years after a chickenpox infection if the dormant virus reactivates. Older adults and those with weakened immune systems are at higher risk.
Natural Experiments and Causality
Both recent studies are considered “natural experiments,” closely resembling randomized controlled trials, which are considered the gold standard in medical research. The vaccine eligibility criteria allowed researchers to compare similar groups, isolating the impact of vaccination.
“What makes the study so powerful is the fact that it is indeed practical as a randomized study with a control group – at least too old to be eligible – and an intervention group – those barely young enough to fit the program,” Geldsetzer said.
Further Research Needed
While studies suggest a potential protective effect of shingles vaccines against dementia, nonetheless of the vaccine type, further research is necessary to confirm this link and establish causality. The underlying mechanisms driving this association remain unclear. Geldsetzer’s team hypothesizes that viruses affecting the nervous system may increase dementia risk, but this requires further investigation.
Dr. Maxime taquet, a lecturer at Oxford University who led a 2024 study on the topic, suggests that differences in immune responses between men and women could explain the observed variations in vaccine effectiveness. “Even if it is a hypothesis for now, we believe that they play a role in which we see differences between sexes,” taquet said.
GSK has announced plans to analyze data from approximately 1.4 million older adults in the United Kingdom, comparing vaccinated and unvaccinated individuals to further investigate the potential link.
In another natural experiment,similar to the Welsh policy,the United Kingdom expanded its shingles vaccination program in 2023,offering the vaccine to adults aged 65 and older,while those aged 69 were instructed to wait until they turned 70. Researchers will monitor these groups for the progress of dementia symptoms.
Implications for Alzheimer’s Research
These recent findings regarding shingles vaccination could represent a new avenue for Alzheimer’s research,a field that has faced significant challenges in developing effective prevention strategies and treatments. Alzheimer’s disease is the most prevalent form of dementia, affecting an estimated 7 million peopel in Europe, according to the European Council of the Brain.
“Dementia is a huge burden,” Taquet said. “By properly activating certain immune pathways, we could be able to reduce or even reverse the dementia process.”
“There are many steps between what we know now and reach this goal, but I think it is a promising track in this area,” he added.
