Statins: Large Study Finds No Excess Risk for Most Reported Side Effects

For millions taking medication to lower cholesterol, a new analysis offers significant reassurance. A large-scale study published in , in The Lancet, has found that statins do not cause the majority of side effects commonly attributed to them on medication packaging. The research, involving data from over 150,000 participants, challenges long-held concerns about these widely prescribed drugs.
Cardiovascular disease remains a leading cause of death globally, responsible for roughly a quarter of all deaths in the UK and around 20 million deaths worldwide annually. Statins are a cornerstone of preventative treatment, effectively lowering LDL (“bad”) cholesterol and reducing the risk of heart attack and stroke. However, patient anxieties surrounding potential side effects have often led to hesitancy in starting or continuing statin therapy.
Reassessing the Risks: A Comprehensive Analysis
The study, led by researchers at Oxford Population Health, meticulously examined data from 23 randomized, double-blind clinical trials. These trials compared statin therapy against placebo, or different intensities of statin treatment. The analysis encompassed participants in trials comparing statins to a placebo and participants in trials comparing different statin dosages.
Researchers investigated a broad range of potential side effects listed on statin packaging, including cognitive impairment, memory loss, dementia, depression, sleep disturbances, erectile dysfunction, weight gain, nausea, fatigue, and headache. The findings revealed no statistically significant increased risk for almost all of these conditions among those taking statins compared to those taking a placebo. For example, the incidence of cognitive or memory impairment was in both the statin and placebo groups.
“Statins are life-saving drugs used by hundreds of millions of people over the past 30 years,” said Christina Reith, Associate Professor at Oxford Population Health and lead author of the study. “However, concerns about the safety of statins have deterred many people who are at risk of severe disability or death from a heart attack or stroke. Our study provides reassurance that, for most people, the risk of side effects is greatly outweighed by the benefits of statins.”
A Small Increase in Liver Blood Test Abnormalities
The analysis did identify a small increase in risk – approximately – of liver blood test abnormalities in patients taking statins. However, crucially, there was no corresponding increase in more serious liver conditions like hepatitis or liver failure. This suggests that the observed changes in liver blood tests are unlikely to translate into significant liver problems for most individuals.
Countering Misinformation and Improving Patient Care
The findings are being hailed as a vital step in countering misinformation surrounding statins. Professor Bryan Williams, Chief Scientific and Medical Officer at the British Heart Foundation, emphasized the importance of the study’s authoritative, evidence-based reassurance. “Statins are lifesaving drugs, which have been proven to protect against heart attacks and strokes,” he stated. “Among the large number of patients assessed in this well-conducted analysis, only four side effects out of 66 were found to have any association with taking statins, and only in a very small proportion of patients.”
Professor Williams added that this evidence should help prevent unnecessary deaths from cardiovascular disease and facilitate more informed discussions between doctors and patients. Recognizing which side effects are genuinely associated with statins will also allow physicians to consider alternative treatments when appropriate.
Building on Previous Research
This latest study builds upon previous work from the same research group, which demonstrated that most muscle symptoms are not caused by statins. That earlier research found that statin therapy caused muscle symptoms in only of people during the first year of treatment, with no excess risk thereafter. Previous studies have also indicated that statins can cause a small increase in blood sugar levels, potentially accelerating the onset of diabetes in individuals already at high risk.
The Need for Updated Information
Professor Sir Rory Collins, Emeritus Professor of Medicine and Epidemiology at Oxford Population Health and senior author of the paper, highlighted the need to revise statin product labels. “Statin product labels list certain adverse health outcomes as potential treatment-related effects based mainly on information from non-randomised studies which may be subject to bias,” he explained. “Now that we know that statins do not cause the majority of side effects listed in package leaflets, statin information requires rapid revision to help patients and doctors make better-informed health decisions.”
The trials included in the analysis were all large-scale, involving at least participants, and tracked patient outcomes for a median of nearly five years. The double-blind design – where neither participants nor researchers knew who was receiving the statin or placebo – minimized the potential for bias.
This comprehensive review of the evidence provides a strong foundation for more informed conversations about the risks and benefits of statin therapy, ultimately aiming to improve cardiovascular health outcomes for millions.
