Millions of Americans discontinue statin medications due too muscle-related side effects, and new research from Columbia University suggests a potential cause: statins may bind to a protein within muscle cells, disrupting calcium regulation and leading to pain, weakness, or fatigue.
How Statins Impact Muscle Function
Statins work by inhibiting an enzyme in the liver that produces cholesterol. However, the study reveals that some statins can also interact with a protein called the ryanodine receptor (RyR1) inside muscle cells. This interaction causes the RyR1 channel to leak calcium ions,which are crucial for proper muscle contraction and relaxation. Disrupted calcium levels can lead to muscle dysfunction and the reported side effects.
According to Andrew Marks, chair of the Department of Physiology and Cellular Biophysics at the Vagelos College of Physicians and Surgeons, “It is unlikely that this clarification applies to everyone who experiences muscular side effects with statins, but even if it explains a small subset, that’s a lot of people we could help if we can resolve the issue.”
Prevalence of Statin-Associated Muscle Symptoms
Approximately 40 million adults in the United States take statins to manage cholesterol levels. Of these, roughly 10 percent-around 4 million people-experience muscle-related side effects. The Centers for Disease Control and Prevention estimates that nearly 95 million U.S. adults aged 20 or older have total cholesterol levels higher than 200 mg/dL, making statin therapy a common prescription.
The high rate of discontinuation due to side effects presents a significant public health concern, as consistent statin use is vital for reducing the risk of cardiovascular disease.
Implications for Future Treatment
The Columbia University research, published in the journal Nature, opens avenues for developing statins that don’t interact with the ryanodine receptor, or for identifying patients who are more susceptible to these muscle-related side effects. Researchers are exploring ways to mitigate the calcium leak and potentially allow more patients to safely benefit from statin therapy.
Marks notes that patients who have been prescribed statins and refuse to continue the medication due to side effects should discuss choice cholesterol-lowering strategies with thier healthcare provider.
