Sutter Health, a leading healthcare provider based in ‍Sacramento, California, is enhancing chronic disease management through a new program called Sutter Sync. Launched⁢ in spring 2025, ⁣Sutter Sync utilizes proprietary medical devices that seamlessly integrate with Epic’s electronic health record‍ (EHR) system, enabling patients ‍to track their health from home with greater ease and accuracy.

Real-Time Data Transmission

The Sutter Sync program currently offers ⁢patients devices ⁣for monitoring blood pressure, weight, and blood glucose levels. these devices -⁤ a blood pressure⁤ cuff,scale,and glucometer – automatically transmit readings directly to the patient’s Epic MyChart account and into their extensive health record. Critically, this data transfer occurs in real-time, even when the⁤ MyChart app is closed or the patient’s mobile device is out of range, eliminating the need for separate, device-specific applications.

Rapid Development‍ and Positive Early Results

The concept for Sutter Sync was initially previewed at Epic’s User Group Meeting (UGM) in 2024. Within a year, Sutter Health and Epic collaborated⁤ to transition the idea into a fully functional program for patient use. As of September 22,2025,over ⁢3,000 patients are enrolled ⁤in the blood pressure management component of Sutter Sync,and initial feedback ⁤from both patients and clinicians⁣ has been ‍overwhelmingly positive.

Expanding Program ⁣Scope

While initially focused on hypertension, ⁤Sutter Sync has expanded to support patients managing Type 2 diabetes⁣ and those undergoing prenatal care. This expansion demonstrates Sutter Health’s commitment⁣ to leveraging technology ⁤to deliver more personalized and proactive healthcare⁢ across a broader range of chronic conditions. The integration with Epic’s EHR is expected to improve care coordination and enable more timely interventions based on real-world patient data.
