In some cases, the bacteria can cause primary pneumonic plague, which spreads from person to person via respiratory droplets.

The World Health Organization (WHO) tracks these occurrences, noting that while the disease is rare in the 21st century, it is not extinct. The bacteria persist in rodent reservoirs, allowing the disease to emerge in specific geographic clusters when ecological conditions favor flea populations.

Medical professionals identify three primary clinical forms of the infection: bubonic, septicemic, and pneumonic. Bubonic plague, the most common form, causes swollen lymph nodes called buboes. Septicemic plague occurs when the bacteria enter the bloodstream, and pneumonic plague infects the lungs, representing the most severe and contagious form of the disease.

Yersin’s discovery shifted the understanding of the plague from a miasma-based theory to a germ-based reality, providing the foundation for modern diagnostic and treatment protocols.

Deadly 15th Century Plague That Can Still Infect You Today

Historically, the bacteria caused massive demographic collapses.   • Madagascar: Frequent outbreaks occur, often linked to the movement of rats and fleas in urban and rural settings.
  • Democratic Republic of the Congo: The bacteria persist in wild rodent populations.
  • Asia: Regions such as Yunnan province in China and parts of Mongolia continue to report cases among hunting and farming communities.

Unlike the era of the Black Death, the plague is now treatable with antibiotics. According to public health guidelines, early diagnosis and the administration of streptomycin, gentamicin, or doxycycline can significantly reduce mortality rates. However, the effectiveness of these treatments depends on the speed of administration, particularly in pneumonic cases where the disease progresses rapidly.

The continued presence of Yersinia pestis underscores the relationship between environmental health and human medicine. Public health agencies emphasize that monitoring rodent populations and controlling flea vectors are the primary methods for preventing the spillover of the bacteria into human populations.

While the risk of a global pandemic similar to those of the past is low due to modern medicine and sanitation, the bacteria’s persistence in nature ensures that plague remains a relevant concern for infectious disease specialists and global health monitors.
