Preventing type 2 diabetes should begin as early as conception rather than waiting for abnormal blood glucose levels to appear, according to a perspective article published in Nature Medicine. Led by Andreas Birkenfeld, Medical Director of the Department of Internal Medicine IV at University Hospital Tübingen and Transcampus Professor of Diabetes at King’s College London, an international group of experts argues that current screening strategies miss critical windows for metabolic intervention across a person’s life course.
A Life-Course Approach to Metabolic Vulnerability
Type 2 diabetes accounts for approximately 90% to 95% of all diabetes cases globally, developing through a complex interaction of genetic, environmental, biological, and lifestyle factors. Researchers emphasize that prevention should shift from managing prediabetes toward identifying periods of heightened vulnerability to preserve or restore metabolic health. Rather than focusing solely on preventing progression to diabetes, the authors set a primary goal of achieving prediabetes remission, defined as the restoration and maintenance of normal blood glucose levels, or normoglycaemia.
Ten Priorities for Early Intervention
The international expert panel identified ten core priorities to reshape global diabetes prevention. These priorities include redesigning screening approaches across all life stages, accounting for individual differences, establishing clearer stages of disease progression, and ensuring prevention strategies remain accessible across diverse socioeconomic settings.

Critical Windows Across the Lifespan
According to the study findings, diabetes risk does not remain constant throughout a person’s life. The first potential intervention window begins before conception, as parental metabolic health factors, including obesity, can influence a child’s future metabolic trajectory. Pregnancy introduces another vital phase, during which the body naturally develops insulin resistance to support fetal growth. Women who fail to compensate for this demand may develop gestational diabetes, leaving them with a substantially elevated risk of type 2 diabetes later in life and making the postpartum period critical for metabolic monitoring. Childhood and adolescence represent additional sensitive periods. Increased sedentary behaviors, prolonged screen time, and shifts in diet and activity levels contribute heavily to future risk. Later in life, midlife and menopause bring further vulnerability. For women, hormonal fluctuations during menopause alter body composition and metabolism, which can elevate diabetes risk. The authors suggest that personalized prevention models should integrate family history, weight trajectories, pregnancy history, and emerging biomarkers, while expanding research outcomes to encompass wider health markers such as kidney and cardiovascular function.
