Fertility Treatment Options for Unexplained Infertility Assessed

For couples facing unexplained infertility, a range of treatment options exists, from expectant management to more active interventions. Thes include ovarian stimulation (OS), intrauterine insemination (IUI), OS-IUI, and in vitro fertilisation (IVF) with or without intracytoplasmic sperm injection (ICSI). Researchers conducted a network meta-analysis to compare these treatments and better inform clinical decisions regarding fertility treatments and IUI.
The analysis, encompassing 27 randomized controlled trials with 4,349 couples, sought to determine the effectiveness and safety of each approach. The review considered IVF, OS, and IUI against expectant management.
The study found insufficient evidence to definitively differentiate live birth rates between expectant management and the four active treatments. Assuming a 17% live birth rate with expectant management, the chances following OS ranged from 9% to 28%, IUI from 11% to 33%, OS-IUI from 15% to 37%, and IVF/ICSI from 14% to 47%.
The review indicated that OS might increase the likelihood of multiple pregnancies compared to expectant management or IUI. OS-IUI also likely elevates this risk.Data comparing IVF/ICSI to expectant management regarding multiple pregnancies were inconclusive. If the chance of multiple pregnancy following expectant management/IUI is assumed to be 1%,the chance following OS,OS-IUI,and IVF/ICSI would be 1% to 5%,1% to 5%,and 0% to 6%,respectively.
The certainty of the evidence was rated as low to moderate, primarily due to imprecision stemming from limited sample sizes and heterogeneity across studies.

Further research with larger, more homogenous groups is needed to refine these findings and provide more definitive guidance for couples navigating unexplained infertility.
