Vaping Prevalence and Trends

According to data from Public Health France’s 2024 barometer, of adults aged 18-79 reported vaping, with using e-cigarettes daily – representing over 3 million people. The trend indicates sustained use, with over half () of adult vapers having used e-cigarettes for two years or more. A significant proportion – approximately two-thirds () – continue to smoke traditional cigarettes, and the vast majority () are current or former smokers.
However, usage patterns differ among younger individuals. Approximately one-third of vapers aged 13-17 had never smoked before initiating e-cigarette use. The market has also diversified, with a proliferation of devices, flavors, and practices like “do-it-yourself” (DIY) e-liquid mixing, practiced by half of adult vapers, allowing users to customize ingredients and concentrations.
Motivations for vaping include smoking cessation, lower cost compared to cigarettes, sensory pleasure, and, among pregnant women, a perceived reduction in risks to the developing fetus compared to traditional smoking. Young people are particularly drawn to the marketing and diverse flavors available.
Potential Health Risks, Including Without Nicotine
The report emphasizes that while the health consequences of tobacco use are well-established after decades of research, the long-term effects of vaping are still being investigated. However, the review identified probable (high level of evidence, but not conclusive) and possible (limited evidence) risks.
Cardiovascular effects, such as increased blood pressure and heart rate, are considered probable when nicotine is present. Respiratory risks include the potential development of chronic obstructive pulmonary disease (COPD). The use of e-cigarettes may also have a possible carcinogenic effect. Potential cardiovascular and respiratory developmental effects on a fetus were identified, even with nicotine-free products.

Concerns About Aerosol Composition
Although vaping doesn’t involve combustion, the process generates complex aerosols exposing users to toxic substances present in e-liquids. The absence of combustion does not eliminate these exposures, and the French National Agency for Food, Environmental and Occupational Health Safety (Anses) recommends better informing consumers about the risks associated with vaping practices. They also call for increased manufacturer responsibility regarding device and liquid safety, and stricter regulation of ingredients, potentially including restrictions or bans.
Toxicologist Jérémie Pourchez advises prioritizing products with fewer ingredients, favoring flavorless options, and avoiding DIY e-liquid mixing to prevent potentially harmful interactions.
Philippe Camus, a professor of pulmonology at the University Hospital of Dijon, stresses that the health risks of e-cigarettes cannot be attributed solely to individual modifications of ingredients. He points to a “strong link” between e-cigarette use and acute respiratory distress syndrome, arguing that the industry’s emphasis on individual responsibility doesn’t explain this reality.
Recommendations: Prevention and Responsible Use
Based on the identified risks, Anses recommends combating practices that encourage consumption and normalize vaping. Marketing tactics targeting adolescents, particularly through appealing flavors and products, are a significant concern, as nicotine poses specific risks to their developing brains and can lead to dependence. Effective enforcement of existing restrictions on sales to minors, advertising, and promotion is crucial.
Isabella Annesi-Maesano, a researcher at the National Institute of Health and Medical Research, highlights sufficient warning signs – including risks of COPD and pulmonary fibrosis – to warrant the precautionary principle. She has advocated for including e-cigarettes in expanded smoking bans planned for , noting that over of vapers aged 13-17 have a smoker or vaper in their social circle.
For smokers, e-cigarettes may be considered as a smoking cessation tool, but as a second-line option, under the guidance of a healthcare professional, and as a temporary measure with the ultimate goal of complete tobacco abstinence. Anses emphasizes that the evidence supporting the effectiveness of e-cigarettes for smoking cessation is not as robust as that for scientifically approved methods like Champix or nicotine replacement therapies.
