The Growing Importance of Vitamin K2 and Folate for Cardiovascular Health

As we observe American Heart Month in February 2026, emerging cardiovascular science is increasingly focused on the role of nutritional deficiencies in long-term heart health. Specifically, vitamins K and B9 (folate) are gaining recognition for their potential to support cardiovascular function and address vascular aging.

Cardiovascular conditions often develop gradually, potentially beginning in a person’s 20s and are frequently linked to inadequate intake of essential nutrients. Clinicians at the Cleveland Clinic Foundation have noted an “alarmingly high prevalence of vitamin K deficiency and suboptimal recommended intake” within the US population.

Vitamin K2: Protecting Arteries and Bones

Research suggests that increased intake of vitamin K, particularly vitamin K2, may be beneficial in protecting both arteries and bones. Vitamin K2 plays a crucial role in regulating calcium deposition, preventing it from accumulating in arteries and soft tissues – a process that can contribute to cardiovascular disease. It achieves this by activating Matrix Gla Protein (MGP), a key inhibitor of vascular calcification. Low vitamin K status is associated with inactive MGP, arterial stiffness, and increased risk of heart failure and cardiovascular mortality.

Studies have demonstrated that increasing vitamin K2 intake can improve arterial elasticity and potentially reduce the risk of coronary heart disease. Specifically, each additional 10 mcg of vitamin K2 (in the forms of MK-7, MK-8, or MK-9) is associated with a 9% lower risk of coronary heart disease and reduced cardiovascular mortality.

MenaQ7® K2, as MK-7, is a particularly well-studied form of vitamin K2, with nearly two decades of research supporting its benefits. Clinical trials have shown that long-term supplementation with MK-7 can improve vascular elasticity and support healthy blood pressure, particularly in postmenopausal women.

Preliminary results from the VitaK CAC trial indicate that two years of MenaQ7® supplementation significantly slowed coronary artery calcification compared to a placebo. Further research is ongoing to evaluate different dosages and their effects on various patient populations.

Folate: Supporting Vascular Function and Beyond

Folate, or vitamin B9, also plays a vital role in cardiovascular health, primarily through its involvement in homocysteine metabolism and endothelial function. Large-scale studies consistently demonstrate an association between higher folate intake and lower rates of cardiovascular disease and all-cause mortality, especially in individuals with metabolic vulnerabilities.

An increase in folate intake has been linked to a 5% lower risk of total cardiovascular disease events and a 10% lower risk of cardiovascular disease mortality, highlighting its potential protective benefits.

However, it’s important to note that the benefits are most pronounced with the active form of folate, 5-MTHF. Many individuals have an enzyme deficiency that limits their ability to convert folic acid, the more commonly consumed form, into 5-MTHF. Quatrefolic® is a well-studied active folate that bypasses this conversion issue, offering quantifiable benefits for cardiovascular health and other areas like women’s health and fertility.

A Synergistic Approach to Heart Health

Vitamin K2 (MenaQ7®) and active folate (Quatrefolic®) work through complementary pathways to support cardiovascular health. Vitamin K2 regulates calcium deposition, ensuring it’s utilized for bone mineralization rather than accumulating in arteries. Active folate supports healthy homocysteine metabolism and vascular function.

Both ingredients have been extensively studied in human trials, demonstrating positive effects on heart health. The combination of these fat-soluble and water-soluble vitamins offers a science-driven approach to addressing vascular aging, prevention, and long-term cardiovascular resilience.

According to Lacey Hall, MS, RD, Head of Medical Affairs with Gnosis by Lesaffre, a significant portion of the population is likely deficient in both vitamin K2 and folate, presenting a substantial opportunity to improve heart health through targeted supplementation. The Council for Responsible Nutrition estimates that increased vitamin K2 use could reduce coronary artery disease events by 15.7% and generate an estimated $9.48 billion in healthcare cost savings between 2022 and 2030.

Approximately 54% of the world’s population does not consume enough folate from food, and a 2025 study revealed that nearly 73% of women of reproductive age have folate levels below those considered necessary for preventing neural tube defects – and for realizing the cardio-protective benefits of adequate folate intake.

Addressing these widespread nutritional deficiencies through diet and, when necessary, supplementation, is becoming increasingly recognized as a critical component of proactive cardiovascular care.
