In an era marked by rising rates of chronic diseases and an increasing focus on preventative healthcare, wearable technology is emerging as a powerful tool for monitoring vital signs in real-time with ever-increasing precision. While offering unprecedented opportunities for proactive health management, it’s crucial to acknowledge the potential for false reassurance and the importance of appropriate medical interpretation.

Italy, in particular, faces significant health challenges, including high rates of childhood obesity, smoking, and a concerning trend towards sedentary lifestyles. Wearable technology – encompassing devices like the Apple Watch, AirPods, and similar offerings from Samsung, Garmin, and Huawei – is evolving beyond mere entertainment accessories into clinical tools capable of monitoring vital parameters while maintaining user privacy through data encryption.

Identifying Atrial Fibrillation

The heart remains a primary focus of this technological revolution. Atrial fibrillation, often described as a “silent killer” due to its frequently asymptomatic nature, affects approximately 7% of the Italian population, many of whom are unaware of their condition until a catastrophic event like a stroke occurs. Professor Stefano Carugo, Director of Cardiology at the Policlinico di Milano, describes these devices as “companions” for health, highlighting the now-official inclusion of single-lead electrocardiogram recording in the European Society of Cardiology guidelines for early identification of common benign arrhythmias, such as supraventricular tachycardias, and, crucially, atrial fibrillation.

Professor Carugo recounts the case of a 65-year-old patient who experienced months of palpitations. “I asked her to acquire an Apple Watch or similar device,” he explains, “to systematically record her ECGs. An important arrhythmia was indeed detected, and after confirmation at the Policlinico’s emergency room, she underwent successful surgery.”

The advantage of using devices like Apple Watch lies in the ability to collect biometric data at home, reducing pressure on already strained healthcare systems. Here’s particularly valuable for conditions requiring continuous monitoring. The ability to independently acquire data like ECGs, blood pressure, and oxygen saturation allows patients to share information with their physicians without frequent hospital visits.

While a standard electrocardiogram performed in a clinic utilizes 12 leads to assess cardiac health and identify potential ischemia or infarction, a single-lead ECG is sufficient for diagnosing arrhythmias.

Hypertension and Potential for Misleading Results

While the detection of atrial fibrillation with Apple Watch is supported by robust clinical studies – including the Apple Heart Study, one of the largest of its kind – the issue of false positives and false reassurances surrounding hypertension monitoring remains a concern. If home tests do not detect abnormalities, a user might feel falsely secure and avoid seeking medical attention. A study published in the British Medical Journal analyzed the function of notifications for hypertension signs and found that 7.7% of participants without hypertension received an inappropriate alert (false positive), while the function correctly identified only 41.2% of those who were actually hypertensive. Due to these uncertainties, the Italian Society of Hypertension currently does not recommend measuring blood pressure with wrist-worn technology.

It’s important to clarify that the Apple Watch does not directly measure blood pressure. Instead, it analyzes data collected from the optical heart rate sensor over a period of at least 30 days to look for signs of potential chronic pathology.

AirPods: Beyond Music, Towards Hearing Health

Alongside cardiac monitoring, advancements in hearing health are being driven by features in devices like AirPods Pro. Dr. Frank Lin, a member of Apple’s Clinical Team and former otolaryngologist, emphasizes that hearing loss – affecting 20% of the global population – is a major modifiable risk factor for dementia and social isolation. He stresses the importance of a comprehensive approach encompassing protection, screening, and care.

“For younger individuals who often listen to music at high volumes to block out external noise, the active noise cancellation in AirPods plays a crucial preventative role, allowing them to maintain safer listening volumes,” Dr. Lin notes. For older adults, features like Conversation Boost address the common problem of difficulty following conversations in noisy environments.

AirPods Pro also introduced an on-device hearing test that can provide an audiogram-level assessment in approximately five minutes, measuring hearing ability in decibels. While authorized by health authorities like the FDA, experts caution that this is a screening tool, not a comprehensive diagnosis. It cannot detect physical obstructions like earwax or inflammation, nor analyze sound transmission via bone conduction – functions that remain the exclusive domain of a medical professional. However, the test represents a valuable first-level screening for individuals who might not otherwise seek a medical evaluation.

Monitoring Sleep Apnea

The latest Vital Signs app leverages metrics like resting heart rate and blood oxygen saturation to establish a personalized range for each user. A significant innovation is the detection of sleep apnea, utilizing the accelerometer to monitor breathing disturbances over a 30-day period. The Apple Watch can flag signs of moderate to severe sleep apnea, a frequently underdiagnosed condition with serious cardiovascular implications.

The Risk of Overcontrol

However, the potential for overcontrol and stress stemming from the medicalization of daily life warrants critical consideration. Continuous monitoring of heart rate, steps, physical activity, sleep hours, and other parameters, according to some studies, can fuel insecurities and anxiety, as well as generate unnecessary healthcare costs due to unwarranted check-ups. Exaggerated reliance on technology can also lead to dependence. Physicians may find themselves inundated with ECGs and blood pressure readings from patients, requiring them to help interpret the data and prioritize only essential information.
