Why do women experience chronic pain more frequently and for longer durations than men? For decades, this disparity has often been dismissed as subjective, or attributed to psychological factors. However, emerging research is revealing a biological basis for this difference, pointing to hormone-regulated immune cells called monocytes as a key factor.

A study published on February 20, 2026, in the journal Science Immunology, sheds light on this complex issue. Researchers at Michigan State University, led by physiology professor Geoffroy Laumet, investigated the role of monocytes – a type of white blood cell – in pain resolution. Their findings suggest that a specific subset of these cells produces a molecule, interleukin-10 (IL-10), capable of effectively “switching off” the neurons responsible for pain signaling.

“The difference in perception of pain between men and women has a biological basis,” states Professor Laumet in a press release. “It’s not psychological, and you are not fragile. It’s linked to your immune system.” This statement challenges long-held preconceptions about gender and pain.

Understanding Acute vs. Chronic Pain

To understand the significance of this discovery, it’s important to differentiate between acute and chronic pain. Acute pain is a normal response to injury or stimulation, serving as a vital protective mechanism. It’s typically short-lived and resolves as the underlying cause heals. Chronic pain, however, is a more insidious condition. It persists long after the initial injury has healed – typically defined as lasting more than three to six months – and no longer serves a protective purpose. It is considered a disease in its own right, requiring dedicated management.

Currently, pain is often assessed using a scale of 1 to 10, a subjective measure that can be imperfect. However, statistics consistently demonstrate that women report persistent pain more often than men. This difference has historically been attributed to a bias in reporting, but the new research suggests a more fundamental biological explanation.

The Michigan State University team analyzed both male and female mice, and subsequently confirmed their findings in human patients who had experienced trauma, such as from road accidents. The results consistently showed that men have higher levels of IL-10-producing monocytes. Their pain tends to subside more quickly. In women, these cells are less active, leading to prolonged pain and delayed recovery.

The Role of Testosterone

The researchers identified testosterone, a male sex hormone, as a key regulator of monocyte activity. “It promotes the production, by these white blood cells, of the molecule which calms the neurons,” explains Professor Laumet. In male mice, the presence of testosterone significantly stimulates this protective immune response. Conversely, when male sex hormones were blocked, the effect was reversed.

“This study shows that the resolution of pain is not a passive process,” emphasizes Professor Laumet. “It is an active process, driven by the immune system.” This highlights that pain relief isn’t simply the absence of stimulation, but rather an active immune-driven mechanism that actively quiets down pain signals.

Filling a Significant Gap in Understanding

The researchers rigorously replicated their observations through at least five independent tests in mice, consistently achieving the same results. A team from the University of North Carolina at Chapel Hill further validated these findings in human patients.

Elora Midavaine, a chronic pain specialist at the University of California, described the discovery as filling “an important gap” in our understanding of sex differences in pain. “If the differences between the sexes in terms of pain are well documented,” she stated, “the underlying mechanisms still remain largely poorly understood.”

For a long time, women’s pain has been minimized or dismissed as “emotional” rather than biological. This research challenges that prejudice, demonstrating that there are genuine biological reasons why women may experience prolonged pain.

Potential for New Therapies

The findings open the door to potential new therapeutic strategies. The hope is to develop treatments that can stimulate monocytes to produce more IL-10, thereby accelerating pain resolution. The ultimate goal is to create non-opioid therapies that target this specific pathway, acting upstream to prevent pain from becoming chronic.

“Future researchers will be able to build on this work,” predicts Professor Laumet. “This opens new avenues for non-opioid therapies aimed at preventing chronic pain before it sets in.” This research represents a significant step towards transforming our understanding of pain, and, crucially, towards taking women’s pain seriously.
