Xylitol, a low-calorie sugar alcohol commonly found in sugar-free gums, drinks, and baked goods, has been linked to an increased risk of heart attacks and strokes, according to research presented at the European Society of Cardiology’s annual meeting. The investigation, which analyzed thousands of adults across multiple cohorts, suggests that elevated blood levels of the sweetener can enhance blood platelet reactivity and accelerate clot formation.
Blood Xylitol Levels Tied to Cardiovascular Events
Researchers evaluated data from two long-term population analyses: the Canadian Longitudinal Study on Aging (CLSA) and the European Prospective Investigation into Cancer (EPIC)-Norfolk. The combined study population included roughly 18,000 adults with an average age of 61. By analyzing blood samples rather than relying solely on dietary questionnaires, investigators measured circulating xylitol levels directly. During follow-up periods spanning six years for the CLSA group and 30 years for the EPIC-Norfolk group, researchers recorded more than 8,600 major cardiac events, according to study data.
The risk profiles varied between the two cohorts. In the Canadian cohort, participants with the highest circulating levels of xylitol faced a 47 percent higher risk of a major cardiovascular event compared to those with the lowest levels, after adjustments for age, sex, smoking status, and diabetes. In the European cohort, individuals with the highest xylitol concentration showed an 18 percent increase in major adverse cardiovascular events. Mario Witkowski, PhD, a researcher and presenter at Charité University Hospital in Berlin, noted that intake amounts are often difficult to quantify precisely. How Xylitol May Impact Blood Clotting
While the observational design of the studies establishes an association rather than direct causation, scientists have identified a plausible biological mechanism. Research suggests that xylitol activates platelets, which are a sticky substance in blood that stops bleeding,
says Sanjiv Patel, MD, an interventional cardiologist at MemorialCare Heart & Vascular Institute at Orange Coast Medical Center in Fountain Valley, California. If it activates those platelets in the blood, it can form a clot anywhere in the body — including the brain, lungs, and heart.
This builds on earlier laboratory and human investigations. A 2024 study coauthored by Witkowski demonstrated that xylitol directly enhances platelet responsiveness and promotes blood clot formation in human blood. In related research published in the European Heart Journal, led by Stanley Hazen at the Cleveland Clinic, laboratory experiments showed that human platelets incubated with xylitol were more prone to clotting than control samples, while test animals receiving xylitol injections exhibited significantly faster clot formation.
Medical Community Perspective on Sweetener Safety
Xylitol occurs naturally in trace amounts in whole fruits and vegetables, including plums, strawberries, cauliflower, and pumpkin, according to Kelly Johnson-Arbor, MD, a toxicologist at MedStar Health in Washington, DC, and Jamie Alan, PhD, PharmD, an associate professor of pharmacology and toxicology at Michigan State University. The compound also offers recognized dental benefits by preventing the growth of cavity-causing bacteria and plaque accumulation, which explains its widespread inclusion in toothpastes, mouthwashes, and sugar-free mints.
Despite the emerging cardiovascular concerns, health professionals emphasize that the current evidence remains preliminary. Although this association between xylitol exposure and heart disease may sound worrisome, the data supporting it remains limited, and much more research is needed to determine the exact relationship between xylitol and heart health,
Dr. Johnson-Arbor explains. For individuals managing underlying cardiovascular risk factors, however, caution may be warranted. Dr. Patel advises that consumers who do not need artificial sweeteners can eliminate them, though he notes current evidence does not mandate strict avoidance for the general public. Witkowski recommends that consumers prioritize whole foods and minimize heavily processed items where artificial sweeteners are frequently added.
