Navigating the therapeutic relationship with unlikeable patients
My patient had spent several days harassing staff and other patients or miserably skulking in doorways, glaring balefully at passersby. Offers of outpatient therapy and day programs were rejected sharply, and insults flowed freely. She accused us of not caring, while mocking our efforts to help. She also wondered why she was so lonely.
“My kids disowned me. So did everyone else, for no reason.”
In an effort to prod her in a more insightful direction, I was blunt: “Most adult children I know don’t cut off their mother for no reason.” She became silently sheepish at that. “Well. I’m old now. I can’t change the past.”
