In July 2023, the U.S. Food and drug Administration (FDA) granted traditional approval to Leqembi (lecanemab), an antibody therapy designed to remove amyloid plaques from the brain – a hallmark of Alzheimer’s. This followed earlier accelerated approval and represents a significant step, as it targets the underlying pathology of the disease rather then just managing symptoms.
However, Leqembi is not without limitations. It requires regular infusions and carries risks of side effects, including amyloid-related imaging abnormalities (ARIA), which can cause brain swelling or bleeding. Its clinical benefit, while statistically significant in slowing cognitive decline, is modest, typically slowing decline by 27% over 18 months in clinical trials.
The Amyloid Hypothesis and Beyond
For decades,the “amyloid hypothesis” – the idea that amyloid plaques are the primary driver of Alzheimer’s – has dominated research. Leqembi’s approval lends support to this theory, but it’s increasingly clear that Alzheimer’s is a multifaceted disease.
