Understanding Alzheimer’s Disease and the Need for New Treatments
Alzheimer’s disease,a progressive neurodegenerative disorder,affects millions worldwide. The Donanemab Trial: Design and Key Findings
The Phase 3 TRAILBLAZER-ALZ 2 trial, published in the New England Journal of medicine on September 4, 2025, enrolled 1,736 participants with early symptomatic Alzheimer’s disease and confirmed amyloid and tau pathology. Participants were randomly assigned to receive either Donanemab or a placebo intravenously every two weeks for up to 76 weeks.
The primary outcome measure was the change from baseline in the Clinical Dementia Rating-Sum of Boxes (CDR-SB) score, a standardized assessment of cognitive and functional impairment. Secondary outcomes included changes in amyloid and tau levels, as well as measures of cognitive performance and daily living activities.
The results were compelling.Donanemab demonstrated a statistically significant slowing of cognitive and functional decline, with a 22.3% slowing of disease progression compared to placebo (p < 0.001). Furthermore, approximately 55% of participants treated with Donanemab showed no clinical progression at one year, compared to 29% in the placebo group.
Importantly, the trial also showed a substantial reduction in amyloid plaques, with nearly all participants receiving Donanemab achieving clearance of amyloid pathology. This suggests that Donanemab’s efficacy is directly linked to its ability to remove amyloid from the brain.
How Donanemab Works: Targeting Amyloid and Tau
Donanemab is a humanized monoclonal antibody designed to selectively target a modified form of amyloid beta found in amyloid plaques. By binding to this modified amyloid, Donanemab triggers the immune system to clear the plaques from the brain.
The trial data also suggests a potential impact on tau pathology. While Donanemab primarily targets amyloid, the reduction in amyloid burden appeared to correlate with a slowing of tau accumulation, a key driver of neurodegeneration. This suggests that clearing amyloid may have downstream effects on tau pathology, offering a more comprehensive approach to disease modification.
Safety Considerations and Adverse Events
While Donanemab demonstrated significant efficacy, it is indeed not without potential risks. The most common adverse events were infusion-related reactions, such as fever, chills, and nausea.More serious adverse events included amyloid-related imaging abnormalities (ARIA), which can manifest as brain swelling or microhemorrhages.
ARIA occurred in approximately 24% of participants receiving Donanemab, compared to 1.7% in the placebo group. Most cases of ARIA were mild to moderate and resolved on their own, but some required temporary discontinuation of treatment. participants were closely monitored for ARIA with regular MRI scans.
