3. Project Budget:
100,000 yuan, the quotation shall not exceed the budget price.
4. Supplier Qualifications:
A legal person, other organization or natural person registered in the People’s Republic of China, with the ability to independently bear civil liability and a business license for the business scope related to this project, and providing goods and services to the purchaser.
Have a good record of paying taxes and social security funds in accordance with the law.
Have the ability to perform the contract.
Have a sound financial accounting system, good reputation, have not been listed as a dishonest debtor by the “Credit China” website, and have no serious illegal or dishonest behavior.
This project does not accept joint bidding.
5. Registration Information:
Business license (copy), ID card of legal representative and authorized person (copy), power of attorney.
Tax and social security fund payment records.
Credit report downloaded from the “Credit China” website within 3 days before the registration date (should include information on administrative licenses, administrative penalties, etc.).
A list of major clients of similar projects served in the past three years, with copies of contracts and invoices.
Letter of commitment to ensure that all information is true and valid.
Written declaration of non-affiliation from the supplier (see attachment).
Please print this announcement and submit it together with the registration.
6. Submit Application Documents and Catch Receive Start and End Date:
From September 6, 2024 to 17:00 on September 12, 2024 (excluding holidays), the hospital will conduct an initial review of qualifications after acceptance and wait for notification.

8. Illustrate:
If the bidder makes false descriptions, false promises, or provides false certification materials during the procurement process, he/she shall bear all responsibilities and consequences resulting therefrom, including but not limited to cancellation of the bidder’s qualification, compensation for all economic losses, inclusion in the blacklist, and prohibition from participating in the bidding for our institute’s projects within three years.
The bidder shall prepare relevant materials as required. If the qualifications and materials do not meet the requirements, our institute has the right to reject the bid. For the bidders who have passed the preliminary review, our institute will notify the time of the internal bidding meeting separately.
Each company can select 1-2 representatives to attend the meeting, and arrive at the procurement center 30 minutes before the start of the in-hospital bidding meeting to verify the identity information of the authorized person on site.
The authorized person shall bring the response documents, unpack them on site, state the bids, and quote the final price in writing.
Different suppliers whose heads are the same person or who have a direct controlling stake or management relationship shall not participate in the bidding for the same package or the negotiation for the same procurement project that has not been divided into packages. Suppliers must provide a statement (see attachment).
