One such emerging therapy, GOLCA-R, shows promise but necessitates a proactive approach to mitigating infection risks, especially neutropenia. In an interview with Pharmacy Times, a clinical expert, Chavez, shed light on the crucial role pharmacists play in this landscape.
Prophylactic G-CSF and Infection Prevention Protocols
The use of growth factors, specifically G-CSF (granulocyte-colony stimulating factor), is anticipated to be a cornerstone of managing patients on GOLCA-R, drawing from the experiences in clinical trials. Chavez emphasized that some patients may indeed require prophylactic antibiotics as part of their infection prevention strategy. This highlights the need for pharmacists to be well-versed in the potential side effects of GOLCA-R and to implement robust protocols for monitoring and managing these risks.
Pharmacist’s Role in Risk Assessment and Patient Education
With the potential for treatment-related adverse events, such as the reported G5 event of pneumonia, pharmacists are pivotal in assessing patient risk and providing comprehensive education. Chavez underscored that pharmacists will be instrumental in counseling patients about infections, with a specific focus on neutropenia.
“Pharmacists should advocate for the use of growth factors, especially at earlier cycles when we see most of the cytopenias,” Chavez stated. This proactive stance can help prevent severe neutropenia and its associated complications. Moreover, Chavez noted that patients who develop neutropenia might require prophylactic antibiotics, and importantly, notable interactions between GOLCA and common antibiotics used for prevention were not observed in early trials. This provides a degree of reassurance for pharmacists when considering antibiotic prophylaxis.
Dose Selection and Future Directions for GOLCA-R
the progression of GOLCA to phase 3 trials, even with higher observed rates of neutropenia at the 0.4 mg dose, is supported by compelling clinical data. Chavez explained that in earlier trials, a correlation was observed between the number of prior lines of therapy and the incidence of cytopenia, particularly in patients who had undergone autologous transplantation. Though, current trials involving GOLCA are focusing on earlier lines of treatment, such as frontline and second-line settings. This shift in patient population is expected to lead to a lower frequency of cytopenias.
