Pharmacist ⁤Scope Expansion Faces Drug Pricing Challenges

Updated June 10, 2025

The expanding scope of pharmacist practice is creating new opportunities, but looming federal policy changes present significant⁢ financial challenges for health systems. Tom Kraus, Vice President of Government Relations, addressed⁣ these issues at the American Society of Health-System Pharmacists (ASHP) 2025 Pharmacy Futures meeting, highlighting both‍ the promise and peril facing ‍the profession.

Kraus noted that state-level legislation is driving much of the positive change. This year alone, 20 states have ⁤passed bills expanding what pharmacists can do, with ⁤some incorporating standard of care models. This pharmacist role ‍evolution is seen as a major step forward.

However, federal actions related to drug pricing and reimbursement threaten ⁣to undermine these gains. Kraus pointed to a ⁣presidential executive order directing agencies⁤ to examine acquisition costs for 340B-covered entities. While the order doesn’t explicitly call ⁤for reimbursement cuts, Kraus warned that it sets the⁤ stage for Medicare to reduce payments to these entities in the future.

Another concern ‍is the potential for site-neutral payment policies. As Congress seeks ways to ⁢fund legislative packages, it may turn to these policies, which would reduce hospital reimbursement for procedures, imaging, and even drug administration. Kraus cautioned that while this would save the government money,‍ it would substantially impact health system finances.

What’s next

Health systems and pharmacists⁤ will need⁤ to ‍closely monitor both state and federal policy changes to navigate the evolving landscape and ⁢advocate for policies that support patient care and financial⁤ stability.
