A new study suggests that insulin resistance could be a meaningful risk factor in the development of aortic stenosis (AS), the most common type of heart valve disease. The research, focusing on men over 45, indicates a potential link between insulin resistance and the condition.
The findings, published in Annals of Medicine, highlight a previously unrecognised connection. Researchers hope that understanding this link will lead to new preventative measures for AS.
Aortic stenosis involves the narrowing of the aortic valve,which restricts blood flow from the heart. This forces the heart to work harder, potentially leading to heart failure and other life-threatening complications. Symptoms can take years to appear and may include chest pain, fatigue, shortness of breath, and palpitations. Established risk factors include age, being male, high blood pressure, smoking, and diabetes.

The study involved 10,144 Finnish men, aged 45-73, participating in the Metabolic Syndrome in Men (METSIM) Study. All participants were initially free of AS. Over an average of 10.8 years, 116 men (1.1%) were diagnosed with AS. Researchers measured biomarkers related to hyperinsulinemia and insulin resistance at the study’s outset.
The research team identified several biomarkers, including fasting insulin and proinsulin, associated with an increased risk of AS. These remained significant even after accounting for other risk factors like body mass index and high blood pressure, and after excluding participants with diabetes or aortic valve malformations.
Using statistical analysis, the researchers pinpointed biomarker profiles indicating that insulin resistance is an self-reliant predictor of AS, regardless of other cardiovascular risks.
“This novel finding highlights that insulin resistance may be a significant and modifiable risk factor for AS,” said Dr. Johanna Kuusisto,lead author from Kuopio university Hospital in Finland.
Kuusisto added that managing metabolic health could be a new approach to reducing AS risk and improving cardiovascular health,given the prevalence of insulin resistance in Western populations. She suggested future studies should investigate whether improving insulin sensitivity through weight control and exercise can prevent the condition.
