Oral Medication Shows Promise in Treating Laryngeal Dystonia

For individuals grappling with laryngeal dystonia (LD), a rare neurological disorder causing uncontrollable vocal cord spasms, daily life can be a struggle. The condition, also known as spasmodic dysphonia, impacts over 50,000 people in the U.S. and Canada. While Botox injections are a common treatment,they prove ineffective for as many as 40% of patients. Now, research offers a new avenue of hope.
A study spearheaded by Mass Eye and Ear researchers, part of the Mass General Brigham system, indicates that sodium oxybate, an oral medication, substantially reduces LD symptoms in patients who experience symptom relief after consuming alcohol. The findings, stemming from a phase 2b clinical trial and published in the journal Annals of Neurology, build upon years of patient anecdotes suggesting alcohol’s positive effect.
The trial involved over 100 participants. Results showed that a single dose of sodium oxybate lead to a notable improvement in symptoms for those with alcohol-responsive LD, without triggering serious side effects. The drug’s efficacy resulted in at least a 16% voice improvement, averaging 41% among alcohol-responsive patients. However, sodium oxybate showed no important difference from the placebo in LD patients whose symptoms were not improved by alcohol.
Kristina Simonyan, MD, PhD, Dr med, vice chair for clinical research at Mass Eye and Ear and professor at Harvard Medical School, emphasized the need for new treatments. “Our trial gives us hope for a new, effective treatment that can be offered to some of these patients,” Simonyan said. She added that there is significant interest from the dystonia community,with many patients inquiring about the drug’s availability.
The study enrolled 106 participants, including 50 with alcohol-responsive LD, resolute through a standardized alcohol challenge test. Patients traveled from the U.S., the U.K.,and Canada to participate. Each patient received a single dose of either 1.5g of sodium oxybate or a placebo over two days. The trial was double-blind, ensuring neither patient nor clinician knew who received the active drug. Voice symptoms were assessed before and after treatment.
The benefits of sodium oxybate in alcohol-responsive LD became apparent roughly 40 minutes after intake, lasting up to five hours. While some patients reported mild side effects like nausea and dizziness, no serious adverse events occurred, and symptoms did not rebound after the drug’s effects wore off.
”Our findings suggest that sodium oxybate can be taken on an as-needed basis,such as before work or a social event,so patients can tailor treatment to their own daily needs and get in control of their symptoms,” said Simonyan.
What’s next
Simonyan’s team plans a phase 3 multi-site clinical trial to further evaluate the drug’s efficacy and safety. Her lab is also using artificial intelligence to identify patients who might benefit from the treatment and to explore alternative treatments for those not responsive to alcohol.
