[메디컬투데이=김준수 기자] The skin around the eyes is thin and moves a lot, so it ages quickly in middle age. In particular, the midface area, which occupies the center of the face, becomes bulging and saggy as the bone structure decreases due to aging, and the orbicularis oculi muscle and the septum under the eyes weaken, giving a tired and aged impression, which is a concern for many people.
The best way to improve this problem is the ‘lifting lower eyelid’ surgery. This surgery simultaneously improves the lower eyelid and mid-face area, and has the effect of improving overall concerns such as bulging and sagging under the eyes.
The lifting lower eyelid surgery basically includes the process of lower eyelid fat repositioning, as it involves cutting the lower eyelid skin, releasing the adhesions in the lower eyelid sulcus, and simultaneously removing or repositioning the bulging lower eyelid fat. In addition, the lifting process is performed to pull up the sagging orbicularis oculi muscle and the fat above the cheekbone and fix it to the periosteum. Finally, the surgery is completed by appropriately removing and suturing the excess sagging skin due to the lifting.

However, this surgery requires a high level of expertise and a detailed approach. Excessive fat removal can cause the lower eyelids to look sunken, and if the amount of skin removed is not taken into account, side effects such as ectropion (the eyelids turn inside out, exposing the conjunctiva) or entropion (the eyelids roll inward) can occur. In addition, to prevent complications that may occur after surgery, it is important to properly fix the sagging orbicularis oculi muscle.
Lifting lower eyelids is highly effective, but as with all surgeries, careful planning and sufficient consultation are important. Especially for middle-aged people, a customized approach that takes into account the loss of skin elasticity and changes in facial structure is essential. Therefore, it is essential to establish a customized surgical plan by comprehensively analyzing each individual’s facial structure and skin condition through consultation with a skilled medical team with abundant clinical experience.
Director Kim Hyeong-rok of New Day Plastic Surgery emphasized, “The concerns of middle-aged people under the eyes are not just skin problems, but complex problems caused by aging, so an accurate diagnosis and detailed surgical plan are necessary.” He continued, “Lifting lower eyelid surgery should be performed with a specialist who understands the fine structure and skin condition of the eye area and has sufficient clinical experience to minimize side effects and obtain satisfactory results.”
