A recent study has revealed a substantially increased risk of retinal events, including detachment, following cataract surgery in patients with a history of retinal tears.The risk, observed in a large analysis of patient data, is more than five times greater than the baseline risk of retinal detachment after cataract surgery, which typically hovers around 1%. The findings, presented at the American Society of Retina Specialists (ASRS) 2025 Annual Meeting, underscore the need for careful patient selection, pre-operative evaluation, and perhaps delaying cataract surgery in certain individuals.

Researchers analyzed data from a large cohort of patients who underwent cataract surgery after previously experiencing a retinal tear.The study focused specifically on phakic retinal tears – tears occurring in eyes that had not yet undergone cataract surgery.

Here’s a breakdown of the key insights:

Elevated Overall Risk: Patients with a prior retinal tear faced a substantially higher risk of retinal events post-cataract surgery. Timing Matters: A delay of six months or more between laser repair of the retinal tear and cataract surgery appeared to significantly reduce this risk, nearly halving the rate of retinal events. Multiple Tears Increase Risk: Individuals with multiple retinal tears before cataract surgery experienced almost three times the rate of retinal events compared to those with a single tear.
Demographic factors: Men were found to have a 25% greater risk, and patients of Asian ancestry showed a higher risk potentially linked to higher rates of myopia (nearsightedness) within that population.
Age Considerations: Patients who developed a retinal detachment after cataract surgery were,on average,diagnosed with the initial tear at age 64,compared to 67 for those with a recurrent tear. They also underwent cataract surgery approximately three years younger (66 vs. 67).

Implications for Clinical Practice

The study’s findings have significant implications for ophthalmologists and vitreoretinal surgeons.Mrinali Gupta, MD, a vitreoretinal surgeon at Retina Associates of Orange County, emphasized the significance of the time interval between laser treatment for retinal tears and cataract surgery.

“The study found that the risk of subsequent retinal break was approximately 40% lower when the time interval between phakic retinal tear treatment and cataract surgery was 6 or more months,” Dr. Gupta stated.Based on these results, clinicians may consider:

Thorough Pre-operative Examination: Referring patients with a history of retinal tear for a comprehensive, dilated retinal exam before scheduling cataract surgery.
Delaying Surgery: Recommending a delay of at least six months post-treatment of phakic retinal tears before proceeding with cataract surgery.
Individualized Risk Assessment: Considering demographic factors like sex and ancestry,as well as the number of prior retinal tears,when assessing a patient’s risk profile.

Study Limitations and Future Research

While the study’s large sample size is a strength, researchers acknowledge limitations inherent in its retrospective design and reliance on registry data. These factors may limit the definitive strength of the findings. Further prospective studies are needed to confirm these results and refine clinical guidelines.
