A new retrospective cohort study utilizing the TriNetX US collaborative network has revealed a potential link between sildenafil use – commonly known by the brand name Viagra – and certain ocular adverse events. The research, which analyzed data from over 129 million patients across more than 70 health systems, focused on men diagnosed with erectile dysfunction (ED).

The study, conducted by researchers seeking to establish more robust evidence than previously available from case reports and smaller studies, defined exposure as documented sildenafil use. A control group comprised of men with ED who had no recorded sildenafil exposure was also analyzed. Researchers carefully excluded individuals with pre-existing ocular conditions, prior histories of specific retinal issues (serous retinal detachment, retinal vascular occlusion and ischemic optic neuropathy), or exposure to other phosphodiesterase type 5 inhibitors (PDE5i) like tadalafil, avanafil, and vardenafil. This meticulous exclusion process aimed to isolate the potential effects of sildenafil specifically.

Phosphodiesterase type 5 inhibitors, including sildenafil, function by increasing blood flow. They achieve this by elevating levels of cyclic guanosine monophosphate (cGMP), leading to vasodilation. This highlights the potential for more prolonged ocular effects, even in healthy men using the medication for the first time without a prescription.
