Thailand Plans ‘One ID Card for Every Treatment’ Pilot Project in 4 Health Zones

The National Health System Development Committee convened its inaugural meeting to discuss the implementation of a pilot project, “one ID card for every treatment,” in four health zones. The objective is to develop a digital health database system that will connect treatment services. The expected launch of this public service is set for January 2024, as a New Year’s gift to the people.

Prime Minister Mr. Settha Thavisin chaired the meeting, accompanied by Vice-Chairman Ms. Paethongthan Shinawatra and relevant representatives from the National Health System Development Committee and other sectors.

Dr. Chonnan Srikaew, Minister of Public Health, emphasized the continuous development of Thailand’s public health system. It must adapt and transform to align with societal conditions, elevating the quality of healthcare across all dimensions for the people. To guide this transformation, the Ministry of Public Health has formulated 13 policies that will be implemented in 2024.

During the meeting, five main topics were presented: “One ID card protects all,” comprehensive cancer care, the Biomedical Center’s role in supporting dependent, housebound, bedridden, or palliative patients, improving access to services in urban areas (especially Bangkok and Chiang Mai), and enhancing mental health care for psychiatric and drug patients.

In the Bangkok area, there are approximately 7.7 million people under the care of three healthcare systems. Additionally, there is a hidden population of 2.2 million individuals. The healthcare services are provided by a total of 112 hospitals, comprising 24 government hospitals and 88 private hospitals. However, some hospitals don’t have enough beds to accommodate patients, leading to overcrowding and the need for additional treatment facilities. The Don Mueang area, in particular, will be upgraded to the Royal Thai Air Force Hospital (Sikan) to address this issue. Furthermore, the Public Health Service Center No. 60 will be upgraded to a specialist outpatient hospital. Rajavithi Hospital 2, under the Department of Medical Services, Ministry of Public Health, will be established as a referral hospital for patients, including mental health care for drug patients.

Currently, there are approximately 166,563 psychiatric patients at risk of committing violence, accounting for 25.4% of all psychiatric patients. Around 42,287 individuals fall into this high-risk category. Among drug patients, the number stands at 129,081, with a 50.5% average risk of committing violence, equivalent to about 65,206 people. Notably, 75% of psychiatric and drug patients exhibit symptoms before the age of 18.

Despite the pressing need, access to quality psychiatric and drug services remains limited, particularly in terms of multidisciplinary mental health psychiatrists.

Dr. Phongsathorn Pokpermdee, Acting Deputy Permanent Secretary of the Ministry of Public Health, highlighted the significance of the “One ID card protects all” policy. This policy aims to integrate health system data with service units at all levels, making it easier for the public to access services. It also seeks to enhance the efficiency and quality of services, accommodating individuals who don’t have access to mobile phones or smartphones. With the implementation of this policy, individuals can utilize one ID card to apply for services. The Ministry will develop data recording systems for hospitals, pharmacies, and clinics, alongside identity verification systems and electronic health records. Additionally, the Ministry will establish electronic systems for medical certificates, prescriptions, appointment scheduling, and telemedicine. Citizens will be able to access these services through applications like LINE and other various platforms.

The Ministry currently possesses information from over 66 million people who have received COVID-19 vaccinations, while information for approximately 35 million individuals is available through the LINE application. The pilot project for the “one ID card for every treatment” initiative will commence in Phrae, Roi Et, Phetchaburi, and Narathiwat provinces in January 2024. Notably, these provinces will have mobile-based medical records, including health records for pregnant women. The traditional pink antenatal care book will be digitized and stored on individuals’ mobile phones. Moreover, digital medical certificates, complete with doctors’ signatures, will be introduced to eliminate paper usage and prevent forgery. Electronic referral forms will be available for individuals who need to collect medication or undergo blood tests. Various hospitals, public health hospitals, and clinics will establish remote treatment systems, enabling the delivery of medicines via postal services. Payments can be made through online channels.

By April 2024, online payment channels for treatment services and medicines will be available to the public. Patients will no longer have to wait in payment queues but can conveniently make payments through their smartphones. Moreover, referral letters will no longer be necessary as hospitals will be connected online, facilitating the exchange of medical history information. To ensure security, a robust system with a unique OTP code will be implemented.

The National Health Security Office (NHSO) will supervise two measures to enhance convenience. Firstly, the helpline number, 1330, will be expanded to provide 24-hour service to address inquiries and concerns from the public. Secondly, NHSO will support the population by adjusting the advance payment system for different medical facilities. Efforts will be made to expedite budget disbursements, with a maximum processing time of three days. For patients requiring hospitalization, NHSO will aim to approve payments within 14 days.
