Circulating Tumor DNA May Guide Immunotherapy Decisions in Limited-Stage Small Cell Lung Cancer

Researchers found that patients with detectable ctDNA after completing initial chemotherapy were significantly less likely to experience prolonged progression-free survival.This suggests ctDNA status could serve as a biomarker to identify patients who might require choice treatment strategies, such as consolidation immunotherapy.
The study utilized next-generation sequencing (NGS) with a 139-gene lung cancer panel, achieving ultra-deep coverage (30,000×) to assess ctDNA. To ensure accurate results, advanced statistical models, including time-dependent Cox regression, were employed to mitigate immortal time bias – a common issue in biomarker studies.
Implications for Future Trials and Clinical Practice
“These findings offer a compelling rationale for integrating ctDNA-based stratification in future LS-SCLC trials and may help guide real-time decisions on the use of consolidation ICIs,” stated Dr. Bi, the lead researcher. Consolidation immunotherapy refers to administering immunotherapy after initial chemotherapy to help maintain remission.
Currently, the standard of care for LS-SCLC involves chemotherapy followed by radiation therapy. However, a significant proportion of patients relapse, highlighting the need for more personalized treatment approaches. ctDNA analysis could perhaps identify those at highest risk of relapse who would benefit most from immunotherapy consolidation.
