Why PET/CT Outperforms CT in Bone Metastasis Detection: Key Insights and Benefits

F-18 FDG-PET/CT is more effective and cost-efficient than CT for locating biopsy sites in suspected bone metastases, according to a study published on November 24 in Cancer Imaging.
In this trial with 273 patients, F-18 FDG-PET/CT showed a higher diagnostic success rate for bone lesions. Lead authors Yujie Chang, MD, and Yifeng Gu, MD, from Shanghai Jiao Tong University, stated that this is the first prospective randomized controlled trial comparing these imaging tests and confirmed F-18 FDG-PET/CT’s superior accuracy and sensitivity.
Bone metastases occur in over 50% of all cancers. Bone is the third most common site for metastasis, following the lungs and liver. Bone biopsy remains the gold standard for diagnosing these metastases. However, there is no agreement on the best imaging method for selecting biopsy sites.
To address this, the researchers studied whether F-18 FDG-PET/CT could identify biopsy sites more effectively than CT. The 273 patients were randomly assigned to undergo either CT or F-18 FDG-PET/CT prior to their biopsies.
In the F-18 FDG-PET/CT group (137 patients), 81 had malignant metastases, and 56 had benign lesions. F-18 FDG-PET/CT identified 80 of 81 malignant cases, missing only three benign cases. This resulted in a sensitivity of 98.8% and specificity of 94.6%.
In the CT group (136 patients), 89 had malignant metastases, and 47 had benign lesions. CT detected 83 out of 90 malignant cases, misclassifying six benign cases as malignant. This gave CT a sensitivity of 92.2% and specificity of 86.9%.
The main conclusion is that F-18 FDG-PET/CT is more suitable for determining biopsy sites for suspected bone metastases.
In terms of cost, F-18 FDG-PET/CT scans averaged 11,428 yuan ($1,575), while CT scans averaged 13,287 yuan ($1,832). The higher CT cost is linked to a greater need for second biopsies due to unsatisfactory initial results (5.15% for CT vs. 2.19% for F-18 FDG-PET/CT).
