Researchers at Penn Engineering have used artificial intelligence to identify previously unreported side effects associated with GLP-1 medications, including the weight-loss and diabetes drugs semaglutide, and tirzepatide. The findings, published April 10, 2026, in Nature Health, indicate that these drugs may be linked to symptoms affecting body temperature and menstrual cycles.

The study involved the AI analysis of more than 400,000 posts on the social media platform Reddit. The data spanned five years of public online discussions from nearly 70,000 users sharing their personal experiences with GLP-1 therapies.

According to the research, the identified symptoms involving menstrual cycles and body temperature have not yet been described in clinical trials or included on official drug labels.

Validating AI-Detected Signals

To verify that the AI was capturing accurate medical signals, researchers cross-referenced the findings with established side effects. About 44% of the users in the study described at least one known side effect, with symptoms of gastrointestinal distress being the most frequently reported.

Sharath Chandra Guntuku, PhD, a research associate professor in computer and information science at Penn Engineering and the study’s senior author, stated that the detection of well-known side effects, such as nausea, confirms the method is picking up real signals.

Guntuku noted that the underreported symptoms serve as leads that came from patients themselves, unprompted, which could potentially be used by clinicians to improve patient care.

Study Limitations and Demographics

The researchers noted that the study population is not representative of the broader general population. Users of Reddit are generally younger, more likely to be male, and are predominantly based in the United States.

Despite these demographic limitations, the study found that the symptoms reported collectively by the users matched the known side-effect profiles of both semaglutide and tirzepatide.

The Role of Patient Communities in Research

The study highlights a gap between the objectives of clinical trials and the lived experiences of patients. While clinical trials are typically designed to identify the most dangerous side effects of a medication, they may fail to capture the symptoms that cause the most concern for patients in their daily lives.
