A Growing Public Health Concern
Ketamine, an anesthetic and narcotic, is increasingly being diverted from legitimate medical use for recreational purposes. This has led to a surge in the availability and consumption of its analogues – O-PCE, DCK, and 2-FDCK – posing a significant and growing threat to public health. These substances mimic the effects of ketamine, often with unpredictable and potentially fatal consequences. The French Observatory of Drugs and addictive Trends explains that at moderate doses, these compounds induce hallucinations and dissociative states, distorting visual and bodily perception. Higher doses can lead to “decorporation” – the feeling of leaving one’s body – or even a “K-Hole,” a state of profound dissociation and loss of consciousness.
Overdoses and Serious Health Risks are Increasing
Poisoning cases linked to these ketamine derivatives have risen sharply in recent years. A report from the Pharmacodependence Addictovigilance (CEIP-A) in Bouches-du-Rhône documented 39 cases between 2017 and 2023: 13 involving O-PCE, 5 with DCK, and 20 with 2-FDCK. Alarmingly, two-thirds of these cases were classified as serious, requiring hospitalization. The Dramas survey has identified six deaths directly related to the consumption of these ketamine analogues.
The risk of overdose is high and potentially fatal, especially when combined with other substances - a common practice known as polyconsumption. These derivatives are often overlooked in standard toxicology screenings,delaying crucial medical intervention in cases of overdose. as the French national Agency for the Safety of Medicines and Health Products (ANSM) points out, this lack of detection can substantially worsen outcomes.

Repeated use followed by abrupt cessation can trigger withdrawal syndrome, characterized by anxiety, sleep disturbances, and potentially severe urinary and liver complications, including renal failure.

Healthcare providers should be aware that altered consciousness, agitation, hallucinations, delirium, or dissociative states – without typical opioid intoxication signs – coudl indicate intoxication with ketamine derivatives.
The challenge of New Psychoactive Substances (NPS)
These ketamine analogues fall into the category of New Psychoactive Substances (NPS), which are designed to mimic the effects of known drugs while circumventing legal restrictions. According to the Lille University Hospital, tracking the use of NPS presents significant epidemiological challenges. Their constant evolution, coupled with difficulties in detection – both in drug samples and biological specimens due to low concentrations and rapid metabolism - makes accurate monitoring a “real challenge” for toxicologists.
