The Ministry of Health and Welfare introduced the current status of home treatment management to reporters at Hallym University Gangnam Sacred Heart Hospital in Yeongdeungpo-gu, Seoul, which is a cooperative hospital for home treatment for COVID-19 on the 18th.

The at-home treatment team set up under the local government distributes at-home treatment kits to patients and guides them on living rules, an emergency contact network in case of an emergency, and how to install a self-quarantine mobile app.

The home treatment kit contains an oxygen saturation meter, a thermometer, antipyretic, general cold medicine, and hand sanitizer.

Medical staff check the patient’s condition by phone, including body temperature, oxygen saturation, and other symptoms, twice a day, at 9 am and 5 pm.

In addition to monitoring, non-face-to-face treatment is available 24 hours a day upon request from patients, and prescriptions are issued when other medications are required.

The oxygen saturation meter included in the kit is used by putting it on your finger, and if the oxygen saturation falls below 94%, it is considered to be hospitalized.

The at-home treatment management team at Hallym University Gangnam Sacred Heart Hospital is currently monitoring about 100 confirmed patients.

“If you monitor about 100 people twice a day, it takes about 2 hours,” said Jae-gap Lee, a professor of infectious diseases at Hallym University, who heads the home treatment management team. The professor in charge contacts the patient and asks them,” he said.

Nurse Seon-mi Kim, who works in the home treatment management team, explained that she had no choice but to ask more questions over the phone because she was not face-to-face with the patient.

Nurse Kim said, “In the hospital, if you have a fever, you can give antipyretic drugs, but (in the case of home treatment) you have to keep asking if there are sore throat, chest tightness, headache, etc. We are definitely checking,” he said.

As a result of monitoring, if the medical staff determines that the condition is severe, an ambulance will be dispatched within 30 minutes. There are three major criteria for judging the severity of the disease: oxygen saturation of 94% or less, breathing difficulties, and decreased consciousness.

According to the Seoul Metropolitan Government’s Infectious Disease Control Division, as of the 17th, the cumulative number of home-treated patients in Seoul was about 11,400. Among them, 46 patients were transferred to emergency because the situation worsened.

The hospital explained that the transfer of the patient is proceeding without any major problems.

Home therapists who are urgently transferred can be admitted to the hospital within the same day as the principle of assigning a bed first.

The city of Seoul announced that it has shortened the transport time by simultaneously issuing an ambulance request and a bed assignment request for rapid bed assignment.

Kim Ji-yeon, head of the at-home treatment planning team at the Central Accident Resolving Headquarters of the Ministry of Health and Welfare, explained, “We have recently strengthened the transfer system to share the situation in real time and make the division of roles clear.”

Professor Lee continued, “If the scope of home treatment is expanded, it will become difficult for one medical institution in a specific district to handle it. We need to consider how we divide the steps.”
