The patient initially presented with symptoms such as fever, cough, and an itchy throat. Upon seeking medical attention, he was admitted to a nearby hospital on June 9, 2023. Over the course of his hospital stay, the patient received Remdesivir intravenously for three days, followed by steroid injections for five days. However, his condition failed to improve, and multiple antibiotics were administered along with additional X-rays. Despite medical interventions, the patient’s fatigue persisted.

On July 4, 2023, the patient sought further medical assistance at Vichaiyut Hospital. The physical examination revealed a fever of 37.6 degrees Celsius, a rapid heart rate of 106 beats per minute, low oxygen levels at 88%, and X-ray results showing white patches in both lungs. Further tests ruled out viral, bacterial, and autoimmune causes, leaving the medical team to conclude that the patient was facing a cytokine storm-induced severe pneumonia, possibly compounded by bacterial infections.

With treatment comprising high flow nasal oxygen and antibiotics, the patient’s condition has gradually improved. Within 10 days at Vichaiyut Hospital, the patient experienced decreased fatigue, resolution of fever and cough, and an improved lung X-ray image. By July 29, 2023, the patient no longer required oxygen support, with oxygen levels stabilizing at 96% and lung X-ray showing further improvement. Follow-up examinations on August 12, 2023, revealed no further need for oxygen supplementation, with blood tests showing no signs of inflammation and another improved lung X-ray.

This case serves as a reminder that even fully vaccinated elderly individuals can still suffer from severe illness caused by the COVID virus. While vaccines remain an effective tool in combating the virus, it is crucial for the elderly population to prioritize receiving their complete vaccination regimen to minimize the risk of severe pneumonia. This incident highlights the importance of ongoing vigilance and adherence to preventive measures even after vaccination.
