The Complex Link Between Sleep and Depression
For individuals grappling with both insomnia and depression, finding effective treatment can be particularly challenging. These conditions often fuel each other, creating a vicious cycle. Recent research published in Neuropsychiatric Disease and treatment on July 12, 2025, offers a promising avenue for improved care: combining the sedative dexmedetomidine with the antidepressant esketamine.

Study Details: A Randomized Controlled Trial
researchers, led by Yitong Ding from Nanjing Medical University in China, conducted a randomized, controlled trial between November 2023 and March 2024. The study involved 78 participants diagnosed with both insomnia (defined by ICD-10 F51.0 criteria and a Pittsburgh Sleep Quality Index, or PSQI, score of 16 or higher) and moderate depression (measured with a MADRS score between 22 and 35). Participants were randomly assigned to receive either dexmedetomidine plus esketamine (DE arm, n=38) or dexmedetomidine plus a saline placebo (DS arm, n=40).

Key Findings: Esketamine Considerably Improves Sleep quality & Reduces Depression
After three days of treatment, both groups experienced improvements in total sleep duration. Though, the group receiving esketamine alongside dexmedetomidine demonstrated significantly greater gains – an average increase of 163.32 minutes compared to 111.53 minutes in the saline group (P < .001).
beyond simply sleeping longer, the esketamine group also experienced:
- Longer and more substantial periods of N3 (deep) sleep
- Improved sleep continuity
- A greater proportion of time spent in REM sleep
Importantly, esketamine was also associated with significant reductions in symptoms of both depression (measured by the SNRS and MADRS scales) and alterations in sleep stage proportions (N2 and REM).
the improvements in sleep were notably linked to improvements in depression scores. researchers observed a negative correlation between changes in MADRS scores and total sleep time, deep sleep continuity, and N3 sleep duration and proportion.
How Does This Work? The Science Behind the Combination
Dexmedetomidine, an α2-adrenergic agonist, works by mimicking the body’s natural sleep processes, promoting delta brain waves associated with deep sleep. A dose of 1.75 µg/kg has been identified as effective for inducing sedation in individuals with insomnia2. Though, its effectiveness is limited in those also experiencing depression.
Esketamine, a potent antidepressant, appears to augment the effects of dexmedetomidine by prolonging both N2 and N3 sleep stages, ultimately increasing total sleep time. The study suggests this combination may also involve an upregulation of serum brain-derived neurotrophic factor concentrations, a protein crucial for brain health and plasticity.
Safety and Tolerability
The study reported no serious adverse events. The most commonly reported side effects – oral dryness, nausea, and vomiting – were mild and resolved quickly.
